INSTRUCTIONS
Volunteer Finger Printing & TB Screening

All volunteers are required by law to be fingerprinted by Livescan and have a TB clearance according to Community Care Licensing of California.  Livescan will bill Day Break directly.  Please follow these steps.

Please call Live Scan at 1-800-315-4507 to set-up your Finger Printing appointment.  A LiveScan form is available for your download here. 

  1. Our ORI# is A0448. Mark the Volunteer Box.  Your job title is "Volunteer."
  2. Leave "Agency Address Set Contributing Agency"  blank.
  3. Complete Personal Information
  4. Your Number (OCA#) is 435200841.  This is our agency license number.  We were licensed in December 2000.

Also, if asked and for your information:

  1. Day Break Respite and Caregiver Support Services of Catholic Charities is our name.  We are located at 5111 San Felipe Rd., San Jose, CA 95135
  2. The account is under the program manager name:  Helena Fox
  3. Livescan will send the bill to Day Break.  You do not pay anything.
  4. The address of Livescan is: 111 N. Market St., 3rd Floor, San Jose.  The office is accessible via light rail.  We will reimburse mileage if volunteer/staff request. 

Please make an appointment with your doctor or medical clinic to get a TB screening.  Please fax or mail a copy of the result to Day Break.  Our fax is: 408-238-7257.
